DENTAL MEMBERSHIP SAVINGS PLAN

Radiant Dentistry has introduced our in-house membership program to help uninsured patients maintain optimal oral health in a cost- effective way. Our goal is to provide financial opportunities for all patients to get the care they deserve. This is a membership opportunity available only at Radiant Dentistry.

Radiant Dentistry Dental Membership Terms

• Membership fee is non-refundable and due in full upon enrollment.
• Cannot be combined with any dental insurance, payment plan, is count or courtesy or credit.
• Any prior balance must be paid in full at time of membership enrollment.
• Membership covers one year starting from the enrollment date.
• Plans do not apply to any treatment prior to enrollment.
• Unused appointments and or benefits will not be refunded or carried over to the next year.
• Plans cover only in house services.
• Payment for treatment is requested in full at time of service.
• Renewal fee is subject to change annual.
• Members must notify Radiant Dentistry 48 Hours in advanced if cancelling appointment.
• A fee (25% of total treatment) may be charged for missed appointments.
